Patient Eligibility / Ages Treated
Patients must be 5 years from diagnosis and cancer free. The clinic will see patients of any age.

Information Services
The clinic provides information on the website, other resources, and a copy of Childhood Cancer Survivor Book.

Clinical Services
The HOPE clinic is multi-disciplinary. The staff include an oncologist, neuropsychologist, dietician, and for bone marrow transplant patients an endocrinologist.

Psychosocial Services
The clinic staff includes social workers, and a neuropsychologist.

Appointment Information / Times and Duration
The Oncology survivor appointments are held on the 3rd Thursday of the month and Bone Marrow Transplant survivor appointments happen on the 4th Thursday of the month, and additional appointments are scheduled as needed.

Patient Information Needed
Diagnosis and treatment history, medical history (post treatment), emotional / dietary questionnaires.

Insurance Information / Financial Assistance
Financial assistance is decided on case by case basis; contact billing dept 303-861-6422.
