Patient Eligibility / Ages Treated
Patients must have completed their cancer treatment before age 20 and be at least 5 years from diagnosis. The clinic will see patients of any age.

Information Services
Patients are provided with a copy of Childhood Cancer Survivor Book.

Clinical Services
Each patient is given a comprehensive physical examination and provided with a detailed summary of the visit including recommendations.

Psychosocial Services
The appointment includes a psychosocial evaluation.

Appointment Information / Times and Duration
The clinic meets every Friday; appointments last 1 to 3 hours.
