Patient Eligibility / Ages Treated
Patients must be pediatric oncology survivors at least 2 years off treatment. The clinic will see patients of any age.

Information Services
The patient is provided with a personalized treatment summary and educational materials for a healthy lifestyle.

Clinical Services
The clinic does a complete history and physical examination.

Psychosocial Services
The clinic has a licensed social worker, child psychologist and educational liaison who meet with each patient. The clinic also has a vocational counselor consultant.

Appointment Information / Times and Duration
The clinic meets five days a week; appointments last 2 hours.

Patient Information Needed
Treatment record, protocol number, protocol schema and flow sheets; assistance in obtaining records is available.

Insurance Information / Financial Assistance
Accept most insurance and offer payment plans through the charity care office.
