Patient Eligibility / Ages Treated

The Center sees survivors of child and adolescent cancers at least five years from diagnosis and two years off treatment. Survivors of any age are welcome.

Services Offered
All survivors are seen by an MD, NP, nutritionist, exercise physiologist and social worker or neuropsychologist for a comprehensive assessment. Treatment summaries and recommendations for follow-up are reviewed in detail at the visit. Referrals are made as needed.

Detailed nutrional and exercise assessment and recommendations are forwarded to survivors after the visit. A summary of the visit is forwarded to all involved physicians identified by the survivor.

Appointment Information (Times clinic is open) Each visit is approximately 2-3 hours in length. The clinic meets every Wednesday.
