Patient Eligibility / Ages Treated
Patients must have had a diagnosis of childhood cancer and be off therapy for 2 years. The clinic will see patients between the ages of 2 to 25, but will provide limited consultation for patients older than 25 years old.

Information Services
The clinic provides educational services for patients and families.

Clinical Services
The patient is given a comprehensive physical evaluation. Additional tests or appointments with specialists are scheduled as needed.

Psychosocial Services
Counseling for social, emotional or financial issues is available.

Patient Information Needed
Diagnosis and treatment history.

Insurance Information / Financial Assistance
Most health insurance plans are accepted. A social worker is available to assist in dealing with insurance or alternative financial arrangements.
